A physical therapy record consists of all information related to the provision of physical therapy services and is a legal requirement. Failure to maintain accurate physical therapy records is deemed to be negligence.
This guideline may be used for curriculum planning, and in internal and external service quality assurance processes. It has been prepared to support the implementation of WCPT’s policy statement on records management: record keeping, storage, retrieval and disposal. It is designed to be relevant to all physical therapists.
